tattoo consent form & non disclosure agreement

 I Agree – I understand that I must present valid government-issued photo ID before receiving a tattoo.

I Agree – If I am a minor (ages 16–17), I understand I must be accompanied by my parent or legal guardian, present valid ID for both myself and my guardian, provide proof of guardianship, and provide notarized consent as required by Florida Department of Health.

I Agree –I will not bring food or drinks into the tattoo area.

I Agree – I will not bring animals into the studio, except service animals with proper documentation.

I Agree – I will not bring anyone under the age of 18 into the tattoo area unless they are receiving a tattoo.

I Agree – I may bring only ONE friend or guest to accompany me during my tattoo session.

I Agree – I will not touch fresh tattoos or remove bandages in the studio.

I Agree – I understand that my tattoo is guaranteed only if I follow all professional aftercare instructions provided by the Tattoo Artist.

I Agree – I understand some body areas (fingers, palms, neck, feet, ankles, elbows, knees, ears) are high-fade zones and not covered by guarantee.

I Agree – I understand guarantees do not apply if I experience excessive bleeding, swelling, or cannot remain still, resulting in early termination of the session.

I Agree – I am 18 years of age or older OR I have valid notarized parental/legal guardian consent.

I Agree – I do not have hemophilia or any other bleeding disorders; I do not carry any blood-borne diseases (including but not limited to HIV or Hepatitis); I am not pregnant; I do not have a heart condition or epilepsy; and I am not under the influence of alcohol, drugs, or any impairing substances.

I Agree – I accept full responsibility for the tattoo and any consequences that may result.

I Agree – I understand inks are considered safe and non-toxic but may cause allergic reactions.

I Agree – I will not copy, reproduce, or share any tattoo designs, sketches, stencils, or reference images without written consent from the Tattoo Artist.

I Agree – I will not use any designs from my session for other tattoo work without written permission.

I Agree – I will not disclose the Tattoo Artist’s business information, pricing, suppliers, booking systems, or client lists.

I Agree – I will not post photographs of designs in-progress without the Tattoo Artist’s consent.

 I Agree – I understand this NDA remains in effect indefinitely unless released in writing.

I Agree – By checking this box and signing below, I acknowledge that my electronic or physical signature has the same legal effect as a handwritten signature. I have read and agree to all terms of this Tattoo Consent Form & Non-Disclosure Agreement. I understand I may request a paper copy at no cost and can withdraw my consent at any time by notifying the Tattoo Artist in writing.
